In addition to these helpful links, the BVCN encourages you to form a health care team made up of your local health professionals: your primary care physician or endocrinologist; a certified diabetes educator; a registered dietician and perhaps a personal trainer or exercise physiologist. This team is designed to serve as a network of support, providing you with the care you need. It is important that your health care team includes a primary care physician. If you do not have a physician, please refer to our Provider Directory  to view a list of local primary care physicians in the BVCN.

Boulder Valley School District and Longmont United Hospital employees and family members with diabetes also have an additional benefit available to them through the Nurse Care Coordinator. She is available to help you compile your health care team and navigate through the numerous resources that are available to you in our region.
